  • Please provide links to five testimonials or upload scanned letters expressly commenting on competence in the specialty (or send us testimonials on letterhead).

  • Do you have a graduate degree in the specialty? Please list the details including the school, graduation date and specialty. This might take the place of requiring 5 testimonials and length of time of practice in the specialty. Please submit all information for evaluation.
  • Please provide 2-3 references. We reserve the right to call these references if needed.
